I understand that treatments offered here are based on clinical judgement without a complete examination in-person. I agree to follow-up with a physician for a direct in-person evaluation and/or seek immediate emergency evaluation if my symptoms worsen and/or do not improve in a timely duration. Communication during a virtual visit may be exchanged via online chat or asynchronous messaging. These methods are by their very nature not as secure as a face-to-face encounter.

By requesting this consultation you acknowledge that personal health information will be communicated in a manner that is subject to hacking and other malicious behavior. Video conferencing is currently not an option with our platform.
